This form is to be used when a student is in disagreement with his or her instructor regarding an assigned course grade. It should be submitted with all fields completed and after consultation with the instructor has not resolved the matter. Do not submit this form if you have not yet consulted with your instructor regarding your concern. Refer to the Undergraduate Bulletin for additional information regarding proper procedures for an Academic Grade Appeal.
The Family Education Rights and Privacy Act (FERPA) is a Federal law that protects the privacy of student educational records. FERPA prohibits the release of your educational records, both financial and academic, to third parties without your written consent, subject to the exceptions specified under FERPA. Students may waive their rights under the Family Educational Rights and Privacy Act (FERPA) and authorize the release of their educational records—including academic records, student account and billing information, and/or financial aid records—to designated third parties (e.g., parents or guardians) by submitting the FERPA Consent Form to the Office of Academic Records. This authorization will remain in effect for the duration of the student’s enrollment at Southern Wesleyan University.
